Here you will find all the service application forms that you may need during your study. You can either apply online, or download a form and send it in to us. All forms are also included in our Student Handbook. If there is one that you need and is not listed below, please contact us.

If you want to change your contact details

You can do this in your My Open Polytechnic details page.

If you want to make a library request

Please visit our library page.

If you want to apply for a Student ID card

You can find the Student ID application form inside the 2013 Student Handbook, which you'll receive when your enrolment is confirmed. You can also download and print the application form here.

If you want to apply to NZQA for award of your National Diploma or National Certificate

Please download and complete the NZQA form at the bottom of this page and return it to the Open Polytechnic. The NZQA application requires a payment of $15.30 for the certificate. If you are paying by cheque it should be made payable to Open Polytechnic.
